CSP to hold physiotherapy benefits event

Welsh therapists have this week launched an initiative to highlight both the cost-effectiveness and the patient benefits that can occur from physiotherapy.

Representatives from the Chartered Society of Physiotherapy (CSP) aim to highlight the effectiveness of treatment from traditional disorders such as musculoskeletal conditions, to more diverse afflictions such a dementia and urinary incontinence.

The CSP will be holding an event this week to showcase its 10 evidence-based Physiotherapy Works briefings, which provide information on the costs of conditions and the contribution of physiotherapy. These briefings include information on the following:

  • Musculoskeletal disorders – Specifically, the focus will be on the success of self-referral trials. An analysis of self-referral in Scotland found that the average cost of an episode of care was £95.48 for a self-referral compared to £126.17 for a GP referral
  • Urinary incontinence – Studies comparing pelvic floor muscle training to medications has shown the training which physiotherapists can deliver is cheaper and more clinically effective
  • Fragility fractures and falls – Studies have shown that etween 1998 and 2008 there was a 32% reduction in patients admitted to hospital due to falls in the home, 27% in falls in residential homes and of 40% in street falls. Over the same period hip fractures decreased by 3.6%
  • Chronic obstructive pulmonary disease (COPD) – A 26% reduction in hospital re-admission has been found as a result of pulmonary rehabilitation delivered post-COPD
  • Critical care – It has been suggested physiotherapy treatment (as part of a multidisciplinary approach) is integral to promoting lung function, reducing the incidence of ventilator-associated pneumonia, facilitating weaning and promoting safe and early discharge from the intensive care unit

The CSP state the intention of the event is to showcase the contribution of physiotherapy in conjunction with the part played in multi-agency and multidisciplinary teamwork.

“As well as clinical effectiveness, services need to be able to demonstrate cost effectiveness but in ways that benefit patients” a spokesperson said.

70% of Welsh GPs on e-referral system

Nearly 70% of GP practices across Wales are now able to send hospital referrals electronically via the Welsh Clinical Communications Gateway.

300 GP practices connected to the system, with plans for the remaining practices to go live with the system by March. Figures suggest that Welsh GPs refer around 650,000 patients a year, and it is hoped that this will soon be done entirely electronically.

The full scale roll-out comes as a consequence of a number of successful pilots of the e-referral system which began in February 2008.

The Welsh Clinical Communications Gateway is based on the Scottish Care Information (SCI) Gateway, which is currently used by 95% of GPs in Scotland. The NHS Wales Informatics Service (NWIS) assert that they worked closely with the developers of SCI in order to tailor the system to Welsh needs.

The e-referral system uses a set of standard templates to create referrals for patients which extracts data from the GP system. NWIS state that the automatic population of these forms with patient demographics means that the e-referral system will enable more efficient and easier handling of patient referrals, additionally removing any issues with postage.

Discharge information can also be sent to the GP, including essential clinical information held on other care systems, for integration with the patient record.

As a result of the system, hospitals will receive a standard set of information about each patient that is more legible and complete, reduce the amount of human error.

NWIS state that their future goal is to integration the gateway with hospital patient administration systems (PAS) to allow electronic workflow to the consultant and the prioritisation of referral letters.

NHS Wales campaign to save essential SLT services

The Welsh NHS has launched a major campaign to safeguard Speech and Language Therapy (SLT) services during a time of real-term cuts to NHS funding.


Almost 20% of the UK population may experience communication difficulties at some point in their lives. In adults, stroke is the most common cause of speech difficulties, with a third of stroke survivors experiencing communication problems as a result. 700,000 individuals with dementia also have speech, language and communication needs, a figure which rises annually due to the ageing population.

In addition, speech, language and communication difficulties are the most common of all disabilities experienced by children.

SLT is thought to be greatly beneficial to patients, helping to remove the social and mental barriers associated with communication disorders and enabling people to develop or regain vital swallowing skills. Early intervention by speech and language therapists has been shown to enable people towards becoming economically active again, lessening the financial hardship for sufferers and their families.

In economic terms, it is believed that SLT services save money for already over-stretched NHS services and calls on other government budgets in terms of benefits claimed.

The Royal College of Speech and Language Therapists (RCSLT)  launched their “Giving Voice” campaign to champion the wide-reaching economic and social benefits of SLT. NHS Wales have now launched their Banerwr project (Welsh for standard-bearer) as part of that campaign, with a banner being created by therapists, students, children and adult service users with messages from across the seven geographical areas of Wales.

The banner includes the personal stories of many individuals and demonstrates problems are widespread. It has travelled the length and breadth of Wales by helicopter and motorcycle before being delivered by boat to AMs in Cardiff Bay by speech therapy students.

The RCSLT is calling on governments and key decision-makers to prioritise SLT services as a cost-saving intervention, not a cost driving service. Dr Alison Stroud, the Wales policy officer at the RCSLT, says the intention is to ensure that mainstream services support the early identification of communication difficulties and allow for timely access to specialist services.

Dr Stroud says she wants public bodies to deliver the planning and sharing of resources across traditional boundaries such as health, social care, education and probation. She states that she believes clinical experts should also be involved in decisions about budget management and cost saving.

“Helping people to find their voice and, in so doing, realise their potential must surely be an investment in society worth making” Stroud said.

In order that SLT services can accurately justify their contribution to the patient journey and their overall outcome, Trusts must seek to implement a system that can capture and report on all contacts and interventions. The Cost of Care report can highlight exactly how much is being spent in Therapies, which services are essential and which can stand to be cut, ensuring that the high demand and successful treatments are not targeted.

NHS failing to record patient Alerts

A report concerning compliance in some Welsh Trusts has indicated large-scale failures in complying with important patient safety alerts.

The report indicates that some health boards have less than 50% compliance with concerns being raised over the use of blood transfusions, oxygen cylinders and delays in medication reflect the work done on compliance.

Information supplied to the charity “Action Against Medical Accidents” (AvMA) indicates that no health board in Wales was 100% compliant with all alerts by the beginning of May this year.

The Welsh Government requires health boards to comply with these safety and clinical directives in a timely way and to record and audit their own compliance. In a statement, they said that the figures shown were a snapshot in time and whilst they are not necessarily incorrect, they do not reflect the work being done across the NHS to ensure patient safety compliance. Namely, they assert that some of the alerts are classified as incomplete because their monitoring is ongoing.

Cwm Taf Health Board, which achieved a 50% compliance, stressed that some of the delays are down to finalising paperwork, which can prove time-consuming. The paper process of recording these alerts often means that information relating to patient safety alerts is static, and that lengthy administrative processes can delay compliance recording.

Medical Director at Cwn Taf Kamal Asaad said that all alerts that have been actioned and given the classification of complete or “ongoing” relates to the status of progressing final documentation or post implementation validation.

Mr Walsh, of the AvMA, said that whilst the effort to comply with patient safety alerts was appreciated by the charity, they are still calling for a concerted effort by clinicians in the NHS in Wales to ensure there is 100% compliance as soon as possible.

As one of the greatest challenges with compliance is the accurate recording of clinical information, to efficiently manage this Trusts could seek to implement an Electronic Patient Record (EPR) system. Rapid Recovery for knee replacements following Physio

The Rapid Recovery programme implemented by clinicians in North Wales has led to halving the time orthopaedic patients spend in hospital following major surgery such as knee replacements.

North Wales has become one of three centres of excellence for Orthopaedic Rapid Recovery in Britain, and the directive helps patients to be walking again within hours of the operation and which could rapidly speed up the time until discharge.

The average hospital stay for knee replacement patients has consequently been reduced from over six days to less than three, with the time being spend instead at the pre-emptive stage with greater focus on pre-operative assessments across a number of services including: physiotherapy and occupational therapy.

The process begins weeks before surgery, when patients are briefed on how to prepare for surgery and are given the expectation of being mobile again within hours of an operation.

According to Neil Windsor from Betsi Cadwaladr University Health Board (BCUHB), the key is mobilising the joints as soon as possible. “This isn’t about rapid discharge – it’s about timely discharge,” he said.

A faster throughput of patients is predicted to lead to increased capacity for the service and also reduce the financial resources associated with this usually costly process.

According to consultant Tony Smith, the programme is also improving patient outcomes by reducing early complication rates and re-admission to hospital for problems such as thrombosis.
